Help support our locker room: Not only will your puck be inscribed with the name you designate on this form, all proceeds from each puck sold will be used to support annual operating costs of the team locker room. For only $200.00 have your name, name of a player or the name of a loved one inscribed on a puck. Each puck will be showcased in a beautiful display case and proudly hung in the team locker room for years to come.
In recognition of your support you will also be acknowledged in the annual Fairport Hockey Program "Pucks of Fame".
